you cant die of carbon monoxide, as that is not a gas that is released from breathing, its carbon dioxide, so as for your question about suffocating, id say no, unless its air tight which a duvet obviously isn't, otherwise you'd wake up sweating like a tramp, ,, so no although it would be hot and very stuffy
